The first union of liberal doctors calls for a reform of tariffs by setting up a classification of the consultations, according to their complexity.
The return to school will be hot between the unions of doctors and the Health Insurance. During a press conference organized on Wednesday, the CSMF (1), the main union of liberal doctors, denounced the blocking of the prices of medical acts in the conventional framework “Because of an unchanged value for years”.
For the Dr Jean-Paul Ortiz, president of the CSMF, the 23-euro consultation of general practitioners would today be “disconnected from the daily reality of medical practices and this is a disrespectful price, given the professional commitment of doctors”.
“Medical practice has totally changed”
The president of the CSMF justified himself by recalling that the medical practice had “totally” transformed in recent years and that the charges for doctors had changed. Just like the salaries of employees in medical offices and the notion of collaborating spouse, an advantage which is “in the process of disappearing”.
In addition, like many doctors, Jean-Paul Ortiz spoke of the demand for care which has changed. “Today, the doctor no longer intervenes for small acts thanks to self-medication. The aging of the population, the emergence of chronic and intricate diseases, of polypathological patients require heavier and longer management by the doctor, ”he described. Today, faced with this new patient, the doctor just has the possibility of increase the initial price a little, but within a very precise framework.
Classify consultations according to their complexity
Concretely, this union is now calling to reshuffle the cards. “We can no longer fight for an additional 1-2 euros per consultation every 4 years. The increases (MPC, MCS, MNO, MCJ …) are minimal and constitute a tariff imbroglio. Clinical medical acts can no longer be measured by a single price. This is why the CSMF will carry in the months to come (within the conventional framework) a totally different and innovative vision: a total redefinition of the medical exercise ”, he announced.
The CSMF will therefore propose a price reform through the establishment of a classification of consultations, according to their complexity “to enhance the work and expertise of the doctor,” said Jean-Paul Ortiz.
With this system, the consultation of an elderly patient with multiple pathologies should, for example, be better remunerated than that aimed at issuing a medical certificate, or renewing a prescription for drugs.
The promise to create 50,000 jobs
And to be heard, Jean-Paul Ortiz may have found a weighty argument. He claims that by giving more resources to doctors, they will be more inclined to hire medical secretaries. “With an envelope equal to the stakes, we will have the means to create 50,000 jobs”, argues this trade unionist. A figure which is not insignificant in a context of economic crisis and constant rise in unemployment.
“The medical practice must also evolve so that the doctor is not parasitized in his activity by increasingly time-consuming administrative tasks. Stop the bureaucracy! », He hammered.
According to him, this administrative overload is a major cause of the deep crisis expressed today by doctors and of the disaffection of the younger generations vis-à-vis liberal practice, in favor of salaried workers.
Finally, the new school year announcements from other unions are also expected. With, without a doubt, one-upmanship in the claims. They take place in a context where professional elections for the renewal of representatives to URPS of Liberal Doctors (2) will take place on October 12, 2015.
The CMSF maintained its call for “civil disobedience”, by not applying the third party payment (waiver of advance fees) for Complementary Health Assistance (ACS) introduced on July 1. “This measure is too complex to implement. We remain in favor of social third-party payment at the discretion of the doctor, ”the union said. He will therefore continue the boycott of this measure by Marisol Touraine at each stage of compulsory application of the generalized third-party payment planned for 2017. “The compulsory third-party payer, the CSMF does not want it and we will resist! », He concluded.
