A decree details the conditions of application of the Shared Medical File. This file will be accessible to the patient and to healthcare professionals.
Health book, medical file, doctors’ files: everything will soon be brought together in a single document accessible online: the Shared Medical File (DMP). The conditions of use of this tool have just been presented in a decree in the Official Journal. The old personal medical file changes its name but keeps its objectives: to promote prevention, quality and continuity of care for patients. It can be opened by the patient himself or by a healthcare professional, but always with the consent of the holder. It is also the patient who can decide to close his file: he does not replace the file kept by health establishments and doctors.
An online file
This will contain information on the identity of the holder, on his state of health (vaccinations, reports of biology or imaging examinations, prescribed treatments, etc.), but also on his choice in terms of organ donation and advance directives. The identity and contact details of the attending physician, legal representatives (for minors or persons under guardianship) and persons of confidence of the holder will complete the whole.
Health data will be updated in real time. Clinics, hospitals, town doctors, healthcare establishments, the same system will apply to everyone. Access will be via a website or using software currently used by professionals and approved by the Caisse Nationale d’Assurance Maladie.
The patient manages his file
Both the attending physician and the patient will have access to all information. One exception, however: when the practitioner considers that information “should not be brought to the attention of the patient without support”. A maximum period of one month is then provided for organizing an ad consultation. In all other situations, the holder is master of his file. It may designate the health professionals authorized to access information, and prohibit access to certain data to certain practitioners.
