“Our health system is today at a crossroads …” It is with these words that begins the tribune signed by Laurent Berger, secretary general of the CFDT, Danièle Desclerc-Dulac, president of the CISS (patient group), Etienne Caniard, president of the Mutualité Française and Luc Berille, general secretary of UNSA.
In this text, the authors make the observation of a health system in full upheaval. Generalized third-party payer, increased role of complementary health and ARS (regional health agencies)… Profound changes will shape the organization of care in France. In the light of these changes, the signatories therefore call for a metamorphosis of the system to face the challenges that await it.
A “new form of tariff opposability”.
And they are many. “Inequalities in access to healthcare, inefficient organization of healthcare coverage, deterioration of working conditions for healthcare workers, etc.”, list the authors, who call first and foremost to “act in favor of the reduction in out-of-pocket expenses “for patients, essentially for” poorly regulated “care – dental, hearing aids, optics, as well as” for care giving rise to excess fees “and” hospital care “. They therefore advocate a “new form of tariff opposability”.
According to the signatories, this emerging health system will have to take better account of “territorial disparities”, as well as “social and tariff inequalities”. “Payment methods more suitable than fee-for-service could be put in place,” they think. New technologies will have a central role to play, as they offer significant “potential”.
Integrate mutuals, unions and patients
“A true partnership between compulsory health insurance, complementary health insurance and beneficiaries of health services” will be the key to this new system, say the signatories, who suggest redefining the “steering of health policies” in this direction. “Patient associations, social partners, mutuals cannot be excluded from a management now provided by the State and health insurance”, they insist.
