I cycle and do Nordic walking, but I can’t make a climb anymore. Then I’m short of breath. Four years ago I did a bicycle test and the results of a cardiac catheterization were also good. But it doesn’t really feel good.

The complaints you describe are, I fear, due to the general deterioration of your condition. The heart and lungs must provide the muscles with sufficient fuel and oxygen and remove the carbon dioxide that is created during combustion. That ability is diminishing.
you can your improve fitness by exercising a lot. If the heart or lungs are malfunctioning, the condition deteriorates quickly, but sometimes something can be done about it. Since you have been examined by the cardiologist before, I suspect that your heart is the weakest link. You could ask your GP again for a referral to a cardiologist to see if there is a treatable problem. You can postpone the decline of the condition, but adjustment is unfortunately not possible.
