Eight serious medical errors
The man, whose first name or age is unknown, had to undergo a cystoscopy, a bladder examination for which a camera is placed in the urethra. There was a mix-up during the procedure and this man’s foreskin was removed. This kind of events has a name: “never events”, literally events that should not happen. These are serious medical errors, which are completely preventable, but have taken place due to certain flaws.
Each year, the National Health Service, the British health organization, publishes the listing of all these events that have occurred over the past twelve months in the various hospitals in the country. In Leicester hospital alone, eight such events have been recorded in a year and a half. For example, a man had surgery that was planned for someone else because the two patients had similar names, x-rays were done on the wrong part of his body, or a hip replacement was put in. the wrong side on a patient.
No explanations, just excuses
The management of the hospital did not wish to answer the questions posed on this case. Director of Safety and Risk, Moira Durbridge, told the British newspaper only: “We remain deeply and sincerely sorry to the patients concerned, and we obviously apologize to each of them”. In total, more than 400 such events took place in the UK between April 2018 and January 2019.
